This form contains the Activities Waiver and the Photo/Video/Audio Release. We need these two forms for each family so that students can participate in PTA sponsored events and have their photos in the yearbook as well as other LFES PTA media.

LFES Yearbook : $24.25 plus tax
LFES PTA is using TreeRing to build the yearbook. That means our yearbooks have a personal touch: Your book includes 2 free pages for you to personalize with your photos and your memories.
Reserve your child’s yearbook now at the early bird price of $24.25 plus tax, and it will be delivered to your child’s classroom at the end of the school year.
- Now through November 1, 2022: early bird pricing
- March 15, 2023: Recognition Ads deadline (celebrate your child’s milestones and personality with a 1/8-page ad that appears in EVERYONE’S copy of the yearbook)
- April 15, 2023: Design deadline for your 2 free personalized pages (these are optional, and appear only in YOUR copy of the yearbook)
